PATIENT RESPONSIBILITY

 

As medicine becomes more complex, patient care requires a team effort.  The more you know about your own health and take an active role in researching your health issues, the better your care will be.  In order to get the most from your medical care, we ask that our patients take responsibility for:

 

Knowing their personal health history and keeping a written medical record that includes:

     

        - Current medical problems

        - Past medical history including hospitalizations

        - Surgical procedures or invasive tests

        - Vaccines and immunizations

        - All current medications including vitamins, herbals

          or supplements

        - Medication allergies or intolerance

        - Preventive health schedule including date of last pap

          smear, mammogram, prostate or colon cancer

          screening.

  

Knowing their family health history for, ideally, two generations in each direction (i.e., grandparents, parents, siblings, children, and grandchildren).

 

Being on time for visits.  Arriving late not only disrupts our schedule, but the schedule of each patient who follows you.  Please be courteous and arrive on time.

 

Notifying us promptly of a cancellation.  Failing to appear for an appointment not only disrupts our schedule, but also harms a patient whom might otherwise have been seen during that period.  All appointment cancellations must be made 24 hours in advance to avoid charges.

Note:  Scottsdale Thyroid Institute sees patients by referral only.  Patients with insurance plans that require a referral MUST have the referral at the time of service.  As Scottsdale Thyroid Institute does not provide primary care, all patients are required to be referred by a primary care provider, even if their insurance does not require it.
